Start here: these are cut intentionally oversized, with a drop shoulder. They are true to size for the relaxed look shown in the photos. If you want a fitted look, size down. Women and people with smaller frames typically size down 1 to 2 sizes.
The most reliable way to choose is not your usual letter size. Lay a sweatshirt you already own flat, measure straight across from armpit to armpit, and compare that number to the flat width table below.

How do I find my size?
Lay a favorite sweatshirt flat, measure armpit to armpit, and compare to our flat width.

If your measurement falls between two of ours, take the smaller one for a closer fit or the larger one for more room over layers.
Does the sherpa fit the same as the cotton?
Close, but the sherpa is cut a little fuller. The table above is the same for both because the two share a grading spec, and the sherpa measures roughly two inches larger around the chest than the numbers show. If you are between sizes in a sherpa, take the smaller one.
The wearable blanket
One size, generously oversized. It is designed to drape over your clothes like a blanket, not to fit close to the body. Fits most adults.

The t-shirt
The t-shirt is cut more oversized than the sweatshirts. A Medium measures 33 inches from shoulder to hem. If you prefer a regular fit, order 1 to 2 sizes down from your usual size.
A full inch-by-inch chart for the t-shirt is not published yet. Until it is, use the shoulder-to-hem figure above and size down if you want a standard fit.
Should I size up so it fits over other clothes?
Usually no. These already run large, and the sweatshirts and hoodies are cut with enough room for a layer underneath at your normal size. Sizing up mainly makes the sleeves longer, which is the opposite of what you want when a nurse is trying to reach your forearm.
The one case for sizing up is if you want to wear it over a bulky garment, or you want the longest possible body length. The wearable blanket is the better answer for maximum coverage.
Should I size up for warmth?
No. Extra fabric volume is not extra insulation, and a looser garment holds less warmth against the body, not more. If you want more warmth, change the fabric rather than the size: the sherpa fleece styles are heavier than the 95% cotton ones, and the wearable blanket is designed to be worn over your clothes in a treatment chair.
